Your first visit consists of an examination, X-rays (if needed), and a detailed plan of treatment. At this time, we will also address any questions and concerns you might have. Please be prepared to provide the following information at the time of your exam.
• Referral slip and X-rays (if applicable)
• List of medications you are presently taking
• List of any allergies to medications or latex
• ID and Dental Insurance information (if applicable)
